This form is to be filled out only by an employer (current/former), teacher, or community service organization leader. It will be reviewed by the Orange County Farm Bureau Scholarship Committee. Please complete and submit this form using any of the methods below no later than July 30.

1. Online. (Preferred method) You may fill out the online form below.
    

2. Email. You may download a digital copy of this form by clicking here. Once downloaded, you can fill it out in Adobe Acrobat, and then email the completed form to ocfb@sbcglobal.net.
    

3. Mail in. You may download the form by clicking here, print it, fill it out in pen, and then mail it to us at:
